The Terms of Reference (TOR) for an investigation provides a specification for exactly what the investigating manager needs to do and how the investigation should be conducted. It includes key information about the roles of the different people involved and the timescales to work to. This template provides some standard wording around the conduct of investigations and the terms of reference for an investigation. The template can be used in conjunction with the 'How to design and manage an HR investigation' guide.
Form to record a disciplinary investigation
Form to record notes of discussions during formal hearings
This template form can be used to make notes of formal hearing discussions, such as investigation or disciplinary hearings. The template can be amended to provide start and end times, including of any reconvened discussions, and identifies the parties present at the hearing.
