Exit interview questionnaire

This document should be given to an employee who has submitted their resignation, in advance of their exit interview. The questionnaire asks the employee to outline their reasons for leaving and to rate their experience of the organisation and their role. There are also sections covering what the employee would like to discuss at their exit interview and confirming handover arrangements. It refers to consent for providing references, if consent is the lawful basis relied on under data protection legislation.
